DIAGNOSTIC – TERMS OF REPORTING

It is the policy of CHART Techno Solutions Private Limited to obtain consent from the patient prior to starting a diagnostic procedure. As a patient you have the right to decline the diagnostic procedure at any stage without prejudicing your future treatment. By accepting and submitting the informed consent form you, the patient or your legal representative, are providing consent to diagnostic procedure provided by CHART Techno Solutions Private Limited.

As a patient, you agree that the information given by you during enrollment and assessment with respect to medical history and other relevant data related to your health are true to your knowledge. You acknowledge that, you have had sufficient time to elaborate the medical history and all questions have been answered satisfactorily to your knowledge. You understand that you are required to inform your doctor of any changes to your health and medication in order to keep your medical records up to date. You also agree that you have been explained the risk and benefit of the procedure / test being performed on you.

You understand the data collected, is entered into a database, and analyzed and that the confidentiality of data will always be adequately secured. You agree not to restrict the use of any data or results that arise from the analysis provided such a use is only for scientific purposes. However, you understand that your identity will not be revealed in any information released to third parties or published without your specific and written permission.

You understand and agree that your technician/nurse uses various devices, equipment, or tools as a part of the
procedure. Any accidental/consequential/incidental injury caused by or during any such diagnostic procedures for any reason whatsoever, cannot be construed as deliberate and no legal action can be taken against CHART Techno Solutions Private Limited, nor its directors, employees, or authorized representatives.

  • The reported results are for information and interpretation of a qualified medical practitioner or such other medical professionals, who understand reporting units, reference interval and limitation of technologies, CHART Techno Solutions Private Limited will not be responsible for any interpretation whatsoever.
  • It is presumed that the test(s) are performed are on the specimen(s)/sample(s) belonging to the patient named or identified. The reported results are restricted to the given specimen(s) in the given circumstances only.
  • The test results may show interlab variation from time to time for the same parameter for the same patient. Assays are performed in accordance with standard procedures. The reported results are dependent on patient preparedness/individual assay methods/equipment used and quality or specimen(s) received.
  • The results of the investigations are influenced by various factors such as sensitivity and specificity of test procedures, drug interactions and quality of samples. Therefore, each test results must be correlated with clinical symptoms and results of other investigations before confirming the diagnosis.
  • The report(s) will only help/guide in arriving at a diagnosis in conjunction with clinic presentation and related diagnosis. Report(s) are to be interpreted, taking into consideration the above mentioned facts.
  • If the collection date is not mentioned on test requisition form, the date of specimen received will be printed by default as the date of collection.
  • CHART Techno Solutions Private Limited uses state of the art equipment, internationally accepted quality standards and declares appropriate environmental condition requisite for diagnostic tests. CHART Techno Solutions Private Limited or its manufacturing, selling or distributing or channel partners are not liable for the test results.
  • Any queries regarding the authenticity of the test results should be verified within three (3) days from the date of sample collection on the request by the same referring clinician.
  • Certain tests may require further testing at additional cost for derivation of exact value, kindly submit the request within 72 hours of sample receipt in the laboratory.
  • Any preliminary report issued will be followed by a final report which appears in bold. All final reports supersede the preliminary report issued earlier for the same registration number on the given date
  • This report is not valid for medico-legal purposes.
  • Neither CHART Techno Solutions Private Limited, nor its employees/representatives assume any liability, responsibility for any loss/impersonation or damage that may be incurred to any person as a result of presuming the meanings or contents of the report.
  • Partial reproduction of this report is not permitted/valid.
  • Claim/s if any, are subject to Hyderabad, Telangana India jurisdiction only.
